The Take Me Home (TMH) program is a community safety initiative led by the Fort Bend County Sheriff’s Office. It is designed to assist law enforcement in identifying and safely returning individuals who may have difficulty communicating or remembering personal information due to conditions such as Autism, Alzheimer’s, Dementia, or Down Syndrome.
The program maintains a secure database of individuals with special needs who reside within the jurisdiction of the Fort Bend County Sheriff’s Office. This database includes a photograph, physical description, emergency contact details, and relevant medical or behavioral information that may aid officers during an encounter. If a registered individual is found unable to communicate or provide identification, law enforcement personnel can quickly access the TMH database to identify the individual and contact their family or caregiver.
The TMH program has been adopted by numerous agencies across the United States and has proven effective in reuniting vulnerable individuals with their loved ones. By reducing the time it takes to identify and assist a lost person, the program plays a critical role in safeguarding those at risk due to medical or cognitive conditions.
